UPDATED PROCEDURES - READ CAREFULLY

INSTRUCTIONS:

To be eligible for the New York state license, you must comply with the following rules and regulations. Upon completion of each level a test is given and then forwarded for grading. After receiving a passing grade you will have the required certificate from each level. You then contact the New York state licensing division for the upcoming dates of their examination. They will furnish you with various dates and locations.

Once you have made your commitment to attend these courses it is of the utmost importance that you abide by that commitment.

In order to confirm your enrollment in the course MBFAA is sponsoring, you must do the following:

1. Completed registration form must be in this office prior to the first day of the class you are taking, along with your check or money order. We also accept major credit cards.

2. You will receive a confirmation from this office upon receipt of completed registration form and your method of payment has cleared.

3. After enrollment, if for some reason you must cancel for that level, you must notify this office 7 days in advance of the scheduled date or you forfeit the fee for that module. Should you notify this office in the time allowed, a fee would be applied in order to cover expenses. You will then be placed on the list for the next class and notified by this office of the dates.

4. If for some reason MBFAA should deem it necessary to cancel the courses, your fee will be refunded or credited to the next scheduled class.
